Position Summary:
Foster and maintain ongoing relationships with family members, service providers, hospitals, courts, education system providers, and other identified resources to facilitate and implement the Wraparound process.
Position Description:
Cultivate flexible, family-focused, community-based responsive servicesDevelop and maintain the Wraparound Team, including coordinating and leading team meetingsCoordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan, through service delivery with providers and community resources; update plan as necessaryEnsure family support and stabilization during crisesProvide and document the initial and ongoing Life Domain AssessmentMaintain all service documentation requirements, evaluation outcome requirements and data as requiredProvide services in a timely manner and in accordance with Plan of Care and/or Crisis PlanUtilize and monitor Flexible Funding and service coordinationObtain weekly reports from subcontracted providersParticipate in after hours on-call responseAttend Program staff meetings, supervision and any other meetings as requiredParticipate in the Agency and Program CQI Peer review processPerform duties to reflect Agency policies and procedures and comply with regulatory standardsMeet Agency training requirementsReport all MUIs to Site Manager and Supervisor immediatelyOther duties as assignedRequired Knowledge, Skills and Abilities:
Knowledge of community-area resources for youth and familiesEffective written and oral communication skills, including meeting facilitationAbility to manage time and multiple priorities effectivelyDemonstrated organizational abilitiesAbility to work effectively with individuals of a diverse backgroundJob Requirements:
Bachelors degree in related field and two years of direct experience or five years of related experienceMust complete High Fidelity Wraparound model training within 12 months of employment.Essential Functions/Physical Demands:
Positional: Driving in accordance with job duties assigned. Frequent walking and use of stairs. Frequent to constant sitting.Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Occasional to frequent reaching, handling and fingering.Sensory: Rare feeling, tasting/smelling and use of color vision. Occasional use of midrange and far visual acuity, depth perception, and field of vision.
