Position: Older Adult Case Manager

Availability of Position: Full time
Schedule: Monday-Friday 9:00-5:00pm. Requires some evenings.

Salary/Grade: $41,000 per year; 9; Exempt

ResponsibilitiesThe Older Adult Case Manager (OACM) is responsible for client assessment, service plan development (if needed), regular contact with the residents to assess and identify aging needs, making referrals to outside social services not provided onsite, assistance in applying for public benefits (when applicable), proper documentation of services provided, promote collaborative relationship between GSC and Property Manager and Older Adult Case Manager (OACM). OACM is also responsible for all service coordination data entry into ETO system and other duties as assigned.
Qualifications
Bachelor’s Degree in Social Work, MSW preferred, and one to two years of experience in the social services field, preferably working with older adults. Activity planning, and coordination experience preferred. Must be computer literate and skilled in Microsoft Word and Excel. Must be able to navigate the Internet. Must have a valid Pennsylvania driver’s license with good driving record. As a Mandated Reporter, must provide proof of having completed training on recognizing and reporting child abuse meeting the training standards of PA Act 153 of 2014 within 30 days of employment with recurrent training required every five years thereafter. Meets the Child Protection Clearances required by PA Act 153 to include PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record Check. Must have an understanding of and appreciation for the Mission of The Salvation Army.
