Description:Eddy Visiting Nurse & Rehab Association in Troy, NY has an opportunity for a full-time RN to triage calls after hours to assist patients and their caregivers with issues or questions!

Calls are handled by our answering service. Those requiring to speak with a nurse are called out to the on-call Triage Nurse. If patient needs a visit triage nurse notifies the on call nurse.

The RN Triage Nurse will (but not limited to):

Triage calls, assisting patients/families telephonically if possibleDetermines if oncall visit RN needs to see patient at home, and contacts RN on call to share information regarding patient needsCoordinates with RN Supervisors, clinicians and schedulers across the agency to prioritize patient needsParticipate in Team educational meetings

Minimum requirements:

NYS RN licensure2 + years of experience as a RN in home care or acute care settingValid driver license and reliable vehicle

Preferred requirements:

4+ year of home care experienceExperience managing chronically ill patients (heart failure, COPD, diabetes, etc)Strong chronic care managementExcellent computer skills and can work with multiple computer applicationsHigh level of communication skills to work with a diverse team across different departmentsMultitask and can accurately prioritize in a very fast pace environmentMaintain the highest level of customer service while completing day to day duties and responsibilitiesExcellent critical thinking skills to problem solve Excellent time management and organizational skills

Pay Range: $33.50 - $46.95

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
