Position Purpose:

Under direct supervision, is responsible for quantitative and qualitative assembly, analysis and re-analysis of all medical records. Verifies that all information and documents are retrieved as requested and notifies appropriate persons of chart deficiencies.  Assures completion of deficiencies, maintains physician incomplete file, and locates and retrieves signed-out records.  Handles and processes internal and external requests for health information as well as incoming calls, mail and courier distribution.  Assures adherence to federal regulation and state law for all disclosures of information. 

Retrieves all charts of discharged patients on a daily basis from the various units and/or departments of the hospital. Verifies the receipt of all IP, Ambulatory and Observation records.

Performs quantitative initial and re-analysis, by assigning deficiencies either in electronic or paper format.

Performs procedures related to filing, and monitoring incom­plete charts, and notifies appropriate physicians and other hospital personnel of chart deficiencies. Correctly assembles all IP, OP and Observation records and initiates media creation, bar code creation, and tracking system.

Assists physicians and other hospital personnel in chart completion and in obtaining other legitimate information as requested. Answers the telephone and responds to telephone and visitor inquiries according to established policies and procedures and routes to appropriate person. 

Responds to requests for disclosure of PHI from physician offices, business office, third party payors, agencies and patients.  Reviews all requests as to their appropriateness, provides only necessary information and assures compliance to state and federal laws and regulations. 

Utilizes Profile-Chart tracking, Profile deficiency, Profile ROI, and Power chart to process the completion of health information records as well as process requests for information.

Initiates actions to retrieve medical records that are checked-out or that are not received in the department. Is proficient in accessing records in storage, maintained in microfilm, or digitally stored in scanfile. Assures desktop dictating machines are loaded and ready for dictation.  Unloads dictating machines following physician use, and routes tapes to the transcription section. Interacts with physicians to answer questions and solve chart completion problems.

Minimum Qualifications:

Must be a high school graduate with business college courses helpful, but not required.

Must have a minimum of one-year related work experience in a hospital, medical record department, physician office, or other health care setting.

Light typing required; computer data entry preferred; copy machine preferred; good communication skills and telephone technique required.​
