Utilizing the nursing process, the nurse navigator provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the surgical continuum. + Job Description Job Description Position Summary: The Oncology nurse navigator is a professional registered nurse with surgical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Utilizing the nursing process, the nurse navigator provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the surgical continuum. Essential Duties: + Educate patients about their disease, prescribed treatment, and side effects of treatment and how to manage them + Answer questions as appropriate and serve as liaison to the physician for additional questions and concerns + Provide clinical resources -- where to seek a second opinion, treatment or testing that may not be available locally, and how to access reliable sources of information + Assess patients for psychosocial distress and referral to a licensed social worker or counselor if more intensive support is needed + Address tangible barriers to care such as finances, housing, transportation, childcare, prescription costs, etc. + Refer patients to the appropriate contact, such as a social worker + Ensure any health information provided is accessible: this may include securing documents printed in the patient’s own language or arranging for certified medical interpretation, either in-person or by phone/videoconference + Function as a member of an interdisciplinary team to assess, secure, and coordinate appropriate services through the continuum of care + Empower patients, families, and caregivers to assume as much responsibility for their care as possible, within the constraints of age, illness, and other factors + In consultation with other members of the healthcare team, promote awareness of clinical trials as appropriate + Support timely scheduling of appointments, diagnostic testing, and procedures + Educate/identify patients for genetic counseling or fertility preservation, and facilitate appropriate referrals Education and Experience: + Graduate of an NLN accredited program; BSN preferred + Valid State of Maine RN license + Ability to prioritize and reprioritize quickly + Ability to develop collaborative relationships both internally and externally + Strong verbal and written communication skills + Ability to work autonomously + Working knowledge of insurance coverage, managed care and community resources Knowledge, Skills and Abilities: + Ability to engage patients and team members utilizing the CMH Experience Standards I am creating a warming, caring, and non-judgmental environment I am actively listening and seeking information I am honest, truthful, and consistent I am respectful, treating all individuals with dignity and empathy I am serving as a role model, taking both initiative and ownership when appropriate I am working collaboratively and demonstrating teamwork I am resilient and adapt to change in positive ways. + Demonstrated ability to direct and triage in a highly fluid dynamic operational environment. + Ability to collaborate with all layers of the management/ administration team.
