Oncology Nurse Navigator -- Cancer Center Beckley -- Beckley-275 Dry Hill Road

Job Summary

  The oncology nurse navigator will facilitate benefits for people at risk for or diagnosed with cancer. These include a shorter time to diagnosis and start to treatment, increase patient and caregiver knowledge, better adherence to recommended care, and improved quality of life. The goal of navigation is to reduce morbidity and mortality by eliminating barriers to timely access to cancer care, which may be financial, psychological, social, logistical, or related to communication, language, literacy, and equity of health care delivery. ( Gordils-Perez et al., 2017, Munoz et al.,2018, National Academies of Science, Engineering, and Medicine, 2018, Temucin &Nahcivan, 2020, Yakzan et al., 2019). Responsibilities Responsibilities From Job · Develop, coordinate, and provide community outreach activities for prevention, education, and health screenings of participants. Includes the development of education for CAMC patients and identification of performance Improvement issues. · The ONN will facilitate care transitions through diagnosis, treatment, survivorship, and end of life care. · The ONN will be competent in the roles of coordination of care. Communication, education, and professional development. · The ONN advocates for adherence to evidence based clinical practice guidelines and pathways and participates in the collection and analysis of patient outcome data to optimize patient and organizational outcomes. · Assumes leadership for the CAMC CARE team Program. As such, drives execution by translating strategic priorities into operational reality. Drives improvement by leading through vision and values for the ONN patient population. Demonstrates the ability to lead. · Accountability of CARE team goals in alignment with CAMC corporate goals. Maintains current knowledge of standards and stays abreast of changes within their area of expertise. · The ONN will serve as a member of various committees. Provides data necessary to determine opportunities, develop plans, and sustains success. Accountable for annual assessments or reporting to meet the accreditation requirements for the Cancer Center. · Interface with physicians, fellows, staff nurses, and other members of the multidisciplinary team to identify and address system processes, which impact the timely facilitation and delivery of care. · Address the issues to provide optimal care. · Provide education and bridge to resources with the use of the pretreatment and survivorships clinic. · Communicate with administration, Survivorship Coordinator, Director, Nurse manager, Clinical management coordinator regarding patient issues. · Collaborate with providers, community resources, to provide timely and resourceful care to the patient population. · PHYSICAL REQUIREMENTS 67634NNONNAV - Position continued... Education 

 Nursing Degree (ADN or BSN)

Credentials 

 BLS & RN licensure

Experience

3-5 years Oncology experience preferred
