Oncology Pharmacist - Per Diem
Reviews and monitors patient plans (therapy and treatment) for appropriateness based on current clinical and institutional guidelines, as well as patient-specific criteria Identifies treatment issues and medication misuse Assesses all medication orders to ensure they meet established standards and practice guidelines of AHS Cancer Center Advises clinicians on drug selection based on current clinical guidelines to optimize drug therapy based on patient specific considerations such as laboratory values and clinical condition   Adheres to safety requirements in the handling and preparation of chemotherapy and other hazardous agents according to current guidelines (USP 797, USP 800, ASHP, NIOSH) Participates in educational activities and answers drug information questions Participates in the training, education and mentoring of other pharmacists and technicians and precepting of pharmacy students and pharmacy residents Participates in quality, safety and other project work related to oncology service in the outpatient infusion setting
