Primary Function/General Purpose of Position
The Population Health Operations Coordinator provides project management and basic analytics support to the Population Health team. This position ensures work plans are well developed, deliverable timelines are managed, and communications reach key stakeholders. Supports governance and management meetings to include facilitation, planning and coordinating and follow-up.
Essential Job Functions
Collaborates with leadership teams in the planning, assessment, design and implementation phases for initiative and solutionsCreates tools and processes to assist and support implementation of work plans emerging from projects and monitor/track progress against targetsAssists with formulating and presenting reports and organizes implementation activitiesTranslates leadership initiatives and data into effective workplans and communicationsDevelops, assesses, and makes recommendations to team milestones, workplansProvides in-person support to geographically disperse teamsUtilizes knowledge of system resources and market teams to support implementation plansMaintains reporting calendarProvides data analysis for team initiativesSupports governance and management meetings to include facilitation, planning and coordinating follow-upFacilitates meetings and work sessions of key stakeholdersThis document is not an exhaustive list of all responsibilities, skills, duties, requirements, or working conditions associated with the job. Licensing/Certification
Project Management Professional (preferred)
Education
Bachelor’s degree in healthcare or related field (preferred)
Work Experience
2 years related experience (preferred)
