Outpatient Case Manager - Adult Job Details Job Location Livonia Clinic - Livonia, MI Education Level 4 Year Degree *** $5,000.00 SIGNING BONUS ***   POSITION SUMMARY: The Adult Outpatient Case Manager provides case management services and support to an assigned caseload of adult clients receiving outpatient mental health services. They are responsible for assisting clients with case management and care coordination needs that are goal-oriented and individualized.  They develop an integrated treatment plan in collaboration with the client and other service providers as well as any natural support the client wants to participate in.   EDUCATION AND EXPERIENCE: + Minimum if a bachelor’s degree in a mental health, or related field, from an accredited school. + Minimum of 1 year of case management related experience or combination of field practicum and experience. + Prior experience working in a Community Health setting preferred.   COMPETENCIES AND ABILITIES: + Knowledge of various systems operating in Wayne County (i.e., Community Mental Health, Housing, Transportation, Benefits/Entitlements, Department of Social Services, Court, School).   DUTIES AND RESPONSIBILITIES: + Develop, in a timely manner, a written Integrated Biopsychosocial Assessment, Individual Plan of Service (IPOS), Addendums, Periodic Reviews, Progress Notes, and any other necessary documentation. + Meet with the person in accordance with the amount, scope, and frequency noted in their Individual Plan of Service (IPOS) and additionally, as the person’s preference/ needs dictate in the home and/or other community setting while adhering to Person Centered Planning and the principles of Self-Determination. + Develop/maintain liaison relationships with various community facilities. + Assist the client in advocacy for the development and provision of appropriate community services for the client. + Assist with pro-active planning on behalf of the individual related to potential crises situations, health, and safety concerns and provides crisis resolution support and strategies as needed. + Coordinate discharge planning as needed for hospitalizations + Links, coordinates, and provides monitoring of services as identified in the Individual Plan of Service and as communicated by the person and their natural supports. + Work flexible hours to accommodate the needs of the program and clients. + Responsible for keeping client files in order and up to date. + Participate in and contribute to staff case reviews and staff training in collaboration with mental health therapist. + Develop and maintain a comprehensive database of current community resources for staff and clients.   LICENSING AND OTHER REQUIREMENTS: + Must have reliable mode of transportation Must possess a current valid State of Michigan chauffeur’s driver’s license and automobile insurance that is insurable.
