The Outpatient Physical Therapist Performs patient evaluations, re-evaluations and establishes patient treatment plan based upon the referral from the physician / referral source.  Documents the patients’ care and directs supportive staff regarding treatment process.  Provides leadership to Physical Therapy Assistants, Students, Rehab aides and Volunteers.

MINIMUM EDUCATION

Required Minimum Education: 4 year/Bachelor Degree

Specialty/Major: Physical Therapy

Preferred Education: Post Graduate Degree (Ph.D)

Specialty/Major: Doctor of Physical Therapy

LICENSURE/CERTIFICATIONS (must be non-expired/active unless otherwise stated):

Required: Graduate of accredited program in Physical Therapy with minimum of Bachelor’s degree.  Holds a current Physical Therapy license from the State of Ohio in good standing.  Maintains CPR Certification.

Preferred: Graduate of accredited program in Physical Therapy with Doctorate level degree.  Certification in a specialty area, for example: OCS, SCS, NDT, Lymphedema, Women’s Health, etc.
