Our hospitals, care sites and clinicians are recognized for clinical and operational excellence. **_Outpatient Physical Therapist_** **_Southside Medical Center in Petersburg, VA_** **_Full Time, Days (7AM - 5:30PM; No weekends)_** **_$12K Sign on Bonus eligible_** Seeking a Full time physical therapist for ambulatory, outpatient clinic in Petersburg, VA near the main campus of Southside Medical Center. We are an orthopedic clinic and treat a variety of orthopedic issues, as well as neurological patients such as CVA's, TIA's, and concussions. We have a multi-discipline approach including on-site OT and SLP. We are open Monday through Friday from 7am-5:30pm, no weekends. Job comes with the possibility to work weekends/holidays in the acute setting if desired by applicant, although this is not mandatory. **Primary Function/General Purpose of Position** The incumbent shall be responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in a team environment consistent with the position’s qualifications, professional practices and ethical standards. The incumbent shall also demonstrate accountability for and contribution to performance improvement activities as applicable, problem solving, and productivity. **Employment Qualifications** + The incumbent shall be a graduate of a Doctor of Physical Therapy program, qualified Master’s degree program or four year Bachelor’s degree program approved by the APTA (or foreign equivalent), and hold an applicable and current state license or certification to practice or meet all qualifications to be eligible for traineeship. + Experience working with adult and geriatric populations is desirable. + Experience working in the acute care setting is desirable however is not required. + Current BLS certification required or obtained within ninety days of completing new employee orientation. **Essential Job Functions** **Clinical Area** + Conduct initial assessments. + Develop individualized treatment plans. + Provide age appropriate therapeutic interventions. + Evaluate patient response to treatment. + Document the course of patient care in the electronic medical record including progress made and continuing need for treatment. + Provide patient and family education on tasks and resources that support continued functional independence and document same in the electronic medical record. + Participate in discharge planning as part of the interdisciplinary team.
