Job Summary

Summary
The Social Worker is responsible for performing social work assessments and interventions as needed for inpatients, outpatients and emergency department patients. They are dedicated to patient and family centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social and resource issues that impact individual patients, families and at-risk populations. Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.


Essential Functions
Provide supervision to interns and LCSW's

Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues. Formulates biopsychosocial assessment, disposition and treatment plans.

Provides mandated assessments when abuse is suspected (child, disabled adult, elder) and specialized safety assessment when violence is suspected

Participates in patient care conferences.

Reviews and modifies treatment plans as needed, collaborates with other social workers as needed and serves as a resource.

Participates in discharge planning.

Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.

Strong Critical thinking skills

Prepares and maintains all pertinent records, statistics, and progress notes.

Qualifications

20-40 hours per week

Education
Master's Degree required

Independent licensure required

Can this role accept experience in lieu of a degree? No

Licenses and Credentials

Social Worker, Licensed Independent Clinical [LICSW - MA License]
OR
Licensed Mental Health Counselor [MA License]

Experience

Clinical Experience in a medical setting 2-3 years required

Knowledge, Skills & Abilities

Ability to maintain motivation to achieve goals while dealing with challenges.

Excellent organizational and time management skills.

Excellent crisis intervention skills.

Ability to maintain confidentiality and secure sensitive information.

Identifying complex problems and reviewing related information to develop and evaluate options and implement solutions.
