The Comprehensive Aids Program has an exciting opportunity for a (part-time) Outreach Coordinator (N) position. The incumbent oversees the development and implementation of public education and outreach programs, and assumes responsibility for creating, planning, preparing, and executing varied programs to reach targeted populations. CORE JOB FUNCTIONS + Plans promotional, educational, and other events to increase community awareness. + Researches and prepares reports, records, and other documentation to coordinate and implement public and outreach programs. + Responds to public inquiries about program(s). + Trains personnel to carry out an effective public education and outreach programs. + Distributes educational literature. + Documents daily activities and the number of contacts reached or served through outreach initiatives. + Maintains effective lines of communications with current and prospective clients to ensure program goals and objectives are met. + Promotes services offered to the targeted group. + Adheres to University and unit-level policies and procedures and safeguards University assets. CORE QUALIFICATIONS + High School diploma or equivalent required + Minimum 1 year of relevant experience DEPARTMENT ADDENDUM Department Specific Functions + Establish and maintain working relationships with Department of Health and Community agencies to determine outreach needs of community. + Link newly clients referred by Miami Dade Department of Health to medical and Medical Case Management services. + Outreach to patients at risk or lost to follow-up by assessing adherence to appointments, use of lab results to determine medication adherence and referrals for primary and specialty care providers. + Educational Outreach to patients residing in Miami Dade community, ambulatory clinics, and the hospital setting at time of discharge. + Informational literature will be distributed as well as linkage to case management and other services. + Conduct outreach for patients assigned by providers or the outreach referral database. + Contacted and follow patients to ensure adherence to medical appointments and medication adherence for a period of ninety of more days. + Establish success of linking client to services at mandatory times during process. + Document of encounters that are authorized by grant and enter into computerized billing systems (SDIS) providing pertinent information regarding linkage and retention in care. + Attend required training and meeting by Ryan White agency.
