Outreach Services Associate
**Req Number** 156297 PT 22.5 hrs 8am-4pm The CSP aims to decrease cancer mortality of uninsured residents of Suffolk County by providing access to free breast, cervical and colorectal cancer screening, education and treatment services. This position reports to the Outreach Coordinator and works to increase the numbers of men and women who seek breast, cervical and colorectal cancer screening by developing and implementing evidence-based and evidence-informed public education programs. This position ensures that there is a balance of strategies for educating eligible clients about the importance of early detection and screening, that information is available on the services provided by the partnership and active recruitment of eligible clients for cancer screening; ensuring that the expertise of each partnership collaborator is utilized in order to have greater reach into the communities and to maximize the program's effectiveness; and engaging in outreach activities that support enrollment of the priority population . Additionally this position is responsible for intake and eligibility These individuals determine client eligibility for breast, cervical and colorectal cancer screening and/or diagnostic services. They work with network providers to make appropriate cancer screening appointments for eligible clients and complete required NYSDOH intake/eligibility forms and may provide initial data management. In addition, Intake/Eligibility staff communicates client information to case management staff to ensure timely follow-up of screening results + High School Diploma or equivalent required. + 0-1 years of relevant experience, required. *Additional Salary Detail The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).
