The value you’ll bring to the team:

Provides independent advanced practice nursing care including but not limited to completions of assessment, diagnosis, development of treatment plan including outcome identification, patient/family education, and implementation and evaluation of the treatment plan.

Performs patient rounding with an interdisciplinary team and participates in case conferences.

Performs comprehensive physical, psychological, social, spiritual, and cultural assessments through observation, interview, review of medical records and clinical data inclusive of imaging and lab results.

Evaluates patient’s response and intervenes to ensure optimal patient outcomes

Qualifications

The expertise and experiences you’ll need to succeed:

Palliative care experience desired - inpatient or outpatient

Bachelor’s degree, Nursing- Required

Master’s degree, Nursing- Required

Basic Life Support- American Heart Association - Required

Registered as Advanced Practice Nurse - Required

Registered on the State of Colorado Advanced Practice Registry - Required
