**Summary**
**SUMMARY:** Leads efforts in palliative care, education, training community outreach, and program development in the post-acute settings, including skilled nursing, subacute, and acute rehabilitation units; provides appropriate palliative care treatment, counseling and education of patients and their families; provides a full range of clinical and consultative services to an interdisciplinary team consisting of nurses, mid-level practitioners, and physicians; co-authors and manages grants supporting expansion of palliative care services across the system.
**DUTIES & ESSENTIAL JOB FUNCTIONS:** NOTE: The following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.
1. Addresses issues, orients and trains employees, verifies competency and identifies and suggests way to develop skills; monitors workflow.
2. Assists and supports the palliative team members to pursue projects of their interest, including teaching, and quality improvement initiatives.
3. Consults with other palliative care team members regarding patient progress; maintains current knowledge and competency in area of clinical practice.
4. Develops and implements new clinical programs and assists in developing educational sessions to expand primary palliative care throughout the post-acute settings.
5. Orients and backs up contracting physicians on consult service.
6. Performs other duties as assigned.
7. Provides clinical services for the treatment of patients, groups, and families, evaluates patients and recommends treatment plans.
8. Recommends policy and implements procedures for palliative care for an interdisciplinary team of HCPs and collaborates with other departments to create and sustain interdisciplinary team roles.
9. Responsible for maintaining an active clinical practice in his/her specialty area.
10. Along with program leadership assists with scheduling team members; attends and conducts staff meetings as needed, serves on planning and evaluation committees.

**MINIMUM QUALIFICATIONS:**
Required Education: Successful completion of an approved Nurse Practitioner training program that conforms to the Board of Registered Nursing standards; OR successful completion of a Physician Assistant program of instruction in primary health care approved by the Physician Assisting Examining Committee; OR NCCPA for Physician's Assistant.; graduate of an accredited Nursing program. Specific training in palliative care. Bachelor’s Degree
Required Experience: Demonstrated experience which would provide the knowledge and skill necessary for successful performance as a Mid-Level Practitioner; at least 1 years’ experience working in hospice and/or palliative care including administrative experience and clinical experience in a hospital, palliative, long term care or hospice setting; experience in working in teams and managing interdisciplinary teams that are not hierarchically organized. At least 5 years’ experience working as a Physician Assistant or Nurse Practitioner.
Preferred Experience: experience and training in Geriatric care and/or skilled nursing facility settings, experience working in safety net settings with diverse patient and family populations. Experience in quality improvement and nursing and other clinician education.
Required Licenses/Certifications: for Nurse Practitioners, possession of a current Furnishing Number issued by the California Board of Registered Nursing AND Certification by the American Nurses Credentialing Center (ANCC) OR American Academy of Nurse Practitioners (AANP); for Physician Assistants, a valid Drug Enforcement Agency (DEA) Number AND Certification by the National Commission on Certification of Physician Assistants (NCCPA); possession of a valid license to practice as a Registered Nurse and certification as a Nurse Practitioner issued by the California Board of Registered Nursing; OR possession of a valid Certificate as a Physician Assistant issued by the California Board of Medical Quality Assurance; Certification within one year of start date in Hospice and Palliative Care: Advanced Certified Hospice and Palliative Nurse (ACHPN) by the Hospice and Palliative Nurses Association (HPNA) OR Palliative Medicine and Hospice Care Certificate of Added Qualifications from the National Commission on Certification of Physician Assistants.
Preferred Licenses/Certifications: Advanced Certified Hospice and Palliative Nurse (ACHPN) by the Hospice and Palliative Nurses Association (HPNA) OR Palliative Medicine and Hospice Care Certificate of Added Qualifications from the National Commission on Certification of Physician Assistants; Basic Life Support Certification issued by the American Heart Association; ACLS may be substituted for BLS.
Preferred: Bi-lingual or multi-lingual capability, especially Spanish, Cantonese, Mandarin, Vietnamese, Tagalog, Russian.
