Job Summary
We are seeking a compassionate and skilled Occupational Therapist to join our Home Care team. In this role, you will provide high-quality, patient-centered therapy services to individuals in their homes, helping them regain independence and improve their quality of life.
Key Responsibilities
Conduct comprehensive assessments of patients’ functional abilities, limitations, and rehabilitation needs in their home environment.
Develop and implement individualized treatment plans to address patient goals and improve their ability to perform daily activities.
Provide education to patients and caregivers regarding adaptive techniques, equipment use, and strategies for maintaining independence.
Collaborate with a multidisciplinary team, including physicians, nurses, and other therapists, to ensure cohesive care.
Monitor progress, adjust treatment plans as necessary, and document all services in compliance with professional standards and regulations.
Ensure safe and effective discharge planning and provide recommendations for additional resources or services if needed.

Minimum qualifications
Graduate of an occupational therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Occupational Therapy Association.
Eligible for the National Registration Examination of the National Board for Certification in Occupational Therapy (NBCOT).
Must be able to function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with the agency’s clients and their families
Must accept responsibility for maintaining clinical practice skills, learning and adhering to agency’s policies and procedures on an on-going basis.
Must have a minimum of one (1) year experience or have a preceptor for a minimum of 6-8 weeks.
Home healthcare experience preferred.
Must have current driver’s license and reliable transportation to and from work site.
