Part-Time (24 Hours) Occupational Therapist - Rockford, MI Area
**Employment Type:** Part time **Shift:** Day Shift **Description:** **Day 1 Benefits** **Job Summary** We are seeking a compassionate and skilled Occupational Therapist to join our Home Care team. In this role, you will provide high-quality, patient-centered therapy services to individuals in their homes, helping them regain independence and improve their quality of life. **Key Responsibilities** + Conduct comprehensive assessments of patients’ functional abilities, limitations, and rehabilitation needs in their home environment. + Develop and implement individualized treatment plans to address patient goals and improve their ability to perform daily activities. + Provide education to patients and caregivers regarding adaptive techniques, equipment use, and strategies for maintaining independence. + Collaborate with a multidisciplinary team, including physicians, nurses, and other therapists, to ensure cohesive care. + Monitor progress, adjust treatment plans as necessary, and document all services in compliance with professional standards and regulations. + Ensure safe and effective discharge planning and provide recommendations for additional resources or services if needed. **Other benefits** + Health, dental and vision insurance + Short and long-term disability + 403b + Generous paid time off + Mileage reimbursement + Comprehensive orientation **Minimum qualifications** + Graduate of an occupational therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Occupational Therapy Association. + Eligible for the National Registration Examination of the National Board for Certification in Occupational Therapy (NBCOT). + Must be able to function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with the agency’s clients and their families + Must accept responsibility for maintaining clinical practice skills, learning and adhering to agency’s policies and procedures on an on-going basis. + Must have a minimum of one (1) year experience or have a preceptor for a minimum of 6-8 weeks. + Home healthcare experience preferred. + Must have current driver’s license and reliable transportation to and from work site. + Ability to consistently demonstrate commitment to the Mission and Organizational Code of Ethics, and adhere to the Compliance Program. **Apply Today!!!** **Our Commitment to Diversity and Inclusion** Trinity Health is one of the largest not-for-profit, Catholic healthcare systems in the nation.
