Part Time RN Case Manager - Danvers, MA

Amergis Staffing is seeking a Nurse Case Manager (RN) to work in the community with adults who have severe and persistent mental illnesses. The ideal candidate should possess knowledge of psychiatric medications and treatments, and be comfortable with outreach, case management, and providing support in individuals' homes.
Position Details:
+ Location: Danvers, MA
+ Program: ACCS (Adult Community Clinical Services)
+ Schedule: 20 hours per week, two 10-hour shifts (flexible on hours and days)
+ Start Date: Two weeks from offer
+ Salary: $60/hr
Requirements:
+ Must have a Massachusetts license and reliable transportation for home visits
+ Duties and Responsibilities:
+ Act as a primary case manager for 4-8 clients (clients live in surrounding areas)
+ Provide nursing interventions to individuals
+ Assist in the management of medications and the overall functioning of the nursing department
+ Formulate and regularly update Comprehensive Assessments and Individualized Action Plans (IAPs) for clients on your caseload
+ Participate actively in supervision, staff meetings, in-services, and trainings
+ Transport clients to work sites, meetings, daily activities, appointments, and activities as needed
Qualifications:
+ Must have a reliable vehicle
+ 1 year of experience with mental health/psychiatric patients
+ 1 year of medication administration experience
+ 1 year of knowledge of psychiatric medications and treatments
+ Bilingual skills are a plus
