ThePatient Access Nurseat VNH ensures an accurate and timely referral process and insurance verification for services is carried out, schedules all patient admissions, evaluations and routine appointments, and acts as the initial point of contact for inquiries regarding the team from patients, families, staff and other customers.
The Patient Access Nurse:
* Verifies patient insurance.
* Reviews referral documents to apply clinical expertise to determine ability to admit/resume care in consideration of patient’s individual clinical needs, coverage criteria, and appropriateness for care at home.
* Collaborates with clinical team for timely resolution of referral questions and concerns.
* Monitors recertification/reassessment process to facilitate continued treatment.
* Responds to patient and provider questions regarding the insurance process.
* Maintains working knowledge of home health regulations and requirements for services.
* Assists with various audits and compliance requirements.
* Maintains proficiency in Medicare referral and coding regulations.
* Identifies and routes financial assistance needs to the Financial Counseling team.
* Serves as a communication liaison between the care team and the patient, including calls, electronic messages, and paperwork and fax transmissions from patients, pharmacies and other providers, to ensure a timely response.
* Coordinates electronic messages, including obtaining additional pertinent information from the patient as well as answering items prior to sending to care team.
* Manages providers' daily schedule of patient appointments, working closely with the rest of the care team to adjust as needed for optimum patient care.
* Educates patients about our workflows and process of referrals or other services we provide.
* Maintains workflows including: incoming referrals, scheduling and quality work queues.
Minimum Qualifications:
* Graduate of an accredited LPN/LVN program.
* Active LPN in both Vermont and New Hampshireorone compact.
* Minimum of 2 years in a healthcare setting, home health and/or hospice experience preferred.
* Scheduling experience strongly preferred.
