Job Details As an advocate for the patient, the Patient Advocate assists patients and their family members with their needs The Patient Advocate must possess strong communication and interpersonal skills. This advocate supports and assists the Director of Patient Experience in coordinating empathetic and timely relations between patients and their families throughout the hospital. Supports and promotes patients’ rights as they navigate the health care system. The Patient Advocate collaborates well with other disciplines and is responsible for enhancing the process of communication between the patient/family and the healthcare staff. Shows good judgment in setting priorities to determine the most important and useful areas to focus his or her efforts while consistently striving to provide exceptional patient experiences. Interprets information regarding the patient’s and family’s healthcare needs, including patient education, discharge instructions, procedures, emergency encounters, and assessments Serves as a cultural mediator and educates patients appropriately Translates basic written information as needed and within department guidelines. Serves as a conduit to interpret as accurately and concisely as possible while considering cultural implications and idiomatic language differences Arranging appointments, transportation, and facilitating medication interpreter services when needed Identifies available community-based resources and actively manages appropriate referrals, access, engagement, follow-up, and coordination of services Assist patients in communicating concerns, questions, or other information to staff when patients are not fluent or comfortable speaking English. Track and follow up with prospective patients to meet a wide range of patient needs Maintains statistics of all activities and reports tracking monthly. Physically able to stand for long periods & navigate self throughout the hospital environment. Ensure Compliance with all CHS & SFH standards including the corporate complinca assurance program,HIPAA & IT Security Policy. Demonstrates Professional demeanor in verbal and non-verbal communication, sensitivity ,compassion & appropriate problem solving skills All other reasonable job related duties as required. Salary Range USD $27.00 - USD $35.00 /Hr. The salary range or rate listed does not include any differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
