Job Description
We are seeking a compassionate and dedicated Community Care Assistant to support our ElderONE team and provide exceptional care to our residents. The ideal candidate will assist with personal hygiene, companionship, and communication needs, ensuring a calm and reassuring atmosphere within the community.
ResponsibilitiesWork supportively with ElderONE team members and notify respective team members of any medical or clinical needs.Notify the Community Care Wellness Manager, or designee, and the ElderONE Community Health Nurse of any changes in a resident's condition (physical, emotional, or mental health).Assist new residents in their adjustment to the ElderONE program.Deliver personal hygiene supplies, clothing, and stationery as needed.Meet social needs and maintain continued relationships within the community.Provide a calm, reassuring, and sensitive atmosphere within the convent.Provide companionship and engage in genuine conversation with residents.Follow established support plans to address the non-medical needs of residents.Report changes in physical condition, behavior, appearance, and well-being to appropriate personnel.Maintain the dignity, right to privacy, and confidentiality of residents at all times.Assist in extending hospitality to residents, their families, and guests.Ensure a community/family presence for each individual resident as needed.Facilitate communication with residents' families, friends, and community members.Sort and deliver Community Care department mail.Maintain stock items and report needs.Read, update, and complete various communication logs.Clean wheelchairs, walkers, humidifiers, walker trays, and related equipment according to schedule and as needed.Complete basic office functions such as creating, updating, and posting daily sheets, directories, phone lists, appointment reminders, and similar documents.Offer snacks, beverages, and fill and clean water pitchers.Respond and act accordingly to nurse call notifications and operate the nurse call online platform.Follow Universal Precautions, infection control, and emergency preparedness protocols.Participate in continuing education opportunities, in-services, and staff meetings.Essential Skills2+ years of healthcare experience preferred.Experience in long-term care or geriatrics is a plus.Additional Skills & QualificationsStrong communication and interpersonal skills.Ability to work independently and as part of a team.Compassionate and patient-centered approach.Work Environment
The position is full-time, 40 hours a week, with a set schedule from 7:15 AM to 3:45 PM, including every other weekend on/off. There is an opportunity to pick up extra shifts and overtime as desired. The work environment is supportive and collaborative, focusing on providing high-quality care to residents. Dress code is professional and appropriate for a healthcare setting.
Pay and BenefitsThe pay range for this position is $17.50 - $21.50/hr.
