The Patient Care Advocate is a liaison between the patient, the community, and the healthcare professionals to facilitate access and sustain patient retention throughout the best patient experience. Is required for a Patient Care Advocate to build strong relationships with the community, insurance, healthcare professionals, and other entities that impact the patients' healthcare needs.

Duties and Responsibilities
1. Onboard new patients, introduce the clinic team (VIP Tour), facilitate a welcoming
environment, and provide the necessary information about services offered in the
company and in the Wellness Center.
2. Facilitate first appointment scheduling
3. Work closely with the Clinic Administrator to welcome the new patients on the first
visit
4. Promote Wellness Events
5. Guide patients in navigating the healthcare system
6. Assist patients in removing social and medical barriers.
7. Maintain marketing-related item inventory for offices
8. Helps patients understand their insurance coverage
9. Share information about Medicare, Medicare Advantage, Insurance benefits, and any service differentiator of InnovaCare Health
10. Support patients during care around the basics of Medicare and insurance as well as the benefits of Innovacare with their insurance plan
11. Support reputation and to promote patient participation in the company initiatives
12. Support Patient Experience initiatives by performing outbound calls, answering patients’ questions, posting activities on social media following the Marketing guidelines, and others.
13. Documenting patient complaints and taking necessary steps to resolve them.
14. Plans, coordinates, and tracks attendance od Wellness Center activities.
15. Lead and find resources in the community or internally for activities in the Wellness Center
16. Provide outreach and education to individual patients and groups in different settings and with to distinct cultural and ethnic competencies, linguistic or barriers
17. Facilitate activities to encourage patients in their self-care and promote behavioral changes
18. Support retention strategies, performing survey campaigns to disenrolled patients to capture their feedback and provide service recovery as needed.
19. Support the management team in data collection, timely data entry, reporting, development and evaluation of programs,
20. Assist with patient experience monthly patient experience and retention performance outcomes.
21. Support organizational efforts to drive patient experience and loyalty to ensure organizational alignment
22. Support community engagement activities as needed
23. Support membership growth with established tactics such as “Refer a Friend”
Minimum Required Education, Experience & Skills
