Position Summary / Career Interest:The Patient Care Assistant is an unlicensed staff member who is accountable to and completes delegated tasks from the RN in a caring and culturally sensitive manner, with excellent customer service at all times. Demonstrates competence in the areas of critical thinking, interpersonal relationships, and technical skills related to delegated tasks.
This individual directly supports the program’s nursing staff (clinical nurse navigators, aka CNNs) who have direct responsibility for the contributions of providers, their patient enrollees, and their workforce coordinator partners who are all working together to promote timely achievement of improved health outcomes and enrollees return to full duty. Being at the “hub” of the wheel with accountability for three different, and priority customers (providers, patients and partners in that order) creates an abundance of documentation, coordination and outreach, and this will be increasingly true as ALL enrollees are assigned to the treatment group and gain access to the full suite of wraparound services available via RETAINWORKS. The Administrative CNA will work alongside the CNNs as part of that “hub” supporting the CNN in three critical ways:
1. Documentation. Weekly check-ins by the CNN with each patient assigned to the treatment group (which will be all patients) is their central responsibility to the patient. They address a series of questions documented within REDCap as appropriate and other concerns/topics the patient enrollee raises to their attention during these calls. The ACNA will join weekly calls and perform appropriate documentation as required within REDCap to assist and support the CNN in the case calls are being completed as they drive to their sites to pursue additional enrollments or follow current enrollees as they complete follow-up visits. Additional documentation support may be requested by the CNNs.
2. Coordination. During their enrollment in RETAINWORK throughout an episode of care, a great amount of coordination between the patient, CNN, provider and workforce coordinator are required. The ACNA will be responsible for supporting coordination efforts as requested by the CNNs. This could include confirming appointments, adding them to appropriate calendars, and coordinating meeting among the stakeholders.
3. Outreach. A significant part of the CNNs’ role is identification and facilitation of resources that are unique, but important for each patient enrollee’s success in returning to full duty as soon as they are medically able. The ACNA will support CNN efforts to identify, outreach to and coordinate the acquisition/engagement of resources determined and prioritized for the patient enrollee by the CNNs.

Responsibilities and Essential Job Functions

Must be able to perform the professional, clinical and/or technical competencies of the assigned unit or department.Provides basic nursing skills, including but not limited to: vital signs and pulse oximetry; finger stick blood glucose; ADL’s; recording intake and output; collecting urine, stool, sputum specimens; applying/removing compression hose; SCDs etc.; emptying drains, urinary catheters, NG tubes, and ostomies. Additional skills to meet unit needs.Documents in O2 all tasks, I&O, percentage of meals eaten, etc.Maintains a neat, clutter free and clean patient room and work environment.Maintains a safe patient environment to include High Fall Risk interventions as appropriate for high fall risk patients. This can include but is not limited to: hourly rounds; answering call lights in person, promptly for all patients on the unit.Demonstrates proper hand hygiene, standard precautions, and isolation principles.Cares for the patient in restraints, including ROM and fluids/toileting; reorienting confused patients; reinforcement of patient/family education regarding fall and safety precautions.Promptly notifies the RN of change in vital signs from baseline or expected result, change in mental status or condition, any breakdown in skin integrity or redness of skin, patient report of pain, and any other patient needs, concerns or issues.Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.These statements are intended to describe the essential functions of the job and are not intended to be an exhaustive list of all responsibilities. Required Education and Experience

High School Graduate or GED


Required Licensure and Certification

Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA) within 14 daysCertified Nursing Assistant / Aide - CNA - State Department for Aging and Disability Services (Health Occupations Credentialing) ORMedical Assistant Diploma ORDocumentation of Fundamentals of Nursing Course verifying required basic skills (required at time of hire) ORMinimum of 18 months in a specialty tech position ORCompletion of the Gold Standard Intern Program/Acute Care School with University of Kansas Health System
