Patient Care Coordinator
Case Management Care Coordinator Location: Everett, WA Schedule: 5x8 (7:30 AM – 4:00 PM), every other weekend may be required Department: Case Management (60 Case Managers + 5 Coordinators) Are you a multitasking pro with a passion for helping people navigate the healthcare system? What You’ll Be Doing: + Assisting with referrals to Skilled Nursing Facilities (SNFs) + Initiating insurance authorizations + Arranging transportation for patients + Delivering Medicare regulatory documents + Coordinating communication between patients, providers, insurance companies, and other stakeholders What You Bring: + EPIC experience is a must + High School Diploma or equivalent required; Associate’s Degree in Business or Healthcare preferred + 1+ year in a medical/healthcare setting or 2+ years in a customer service role + Proven ability to deliver high-level customer service in a fast-paced healthcare environment + Comfortable handling confidential data with discretion + Basic knowledge of medical terminology + Insurance authorization experience preferred Why You’ll Love It Here: + Be part of a collaborative, mission-driven team + Work closely with licensed clinical staff + Play a key role in ensuring smooth, safe, and compliant care transitions Ready to Make an Impact? Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
