Responsibilities Job Summary Provides patient related care tasks based on the principles of growth and development across the life span of patients served in his/her independent unit. Assists, observes, and reports patient changes to licensed caregiver and maintains accurate medical record documentation relative to the care delivered. Provides and collaborates direct patient care services under the direct supervisor of a licensed caregiver. Responsibilities may include, but are not limited to personal care, activities of daily living, vital signs, intake and output, ambulation, interventions, and implementation of selected portions of the patient care plan as directed under supervisor of nurse. Assists, observes and reports changes in patient condition to licensed caregiver and maintains accurate medical record documentation in relation to the care delivered. Addresses and/or directs questions or concerns from patients, families, caregivers, or health care team members. Performs other duties as assigned. Qualifications Education High School Diploma or GED Required Experience Completion of Block 1 in Nursing OR Medical Assistant (MA) OR Nursing Assistant (NA) OR Emergency Medical Technician (EMT) OR Behavioral Health Technician (BHT) OR 6 months of experience in the healthcare setting within the last year Required Licenses and Certifications Basic Life Support (BLS) Required
