**Position Overview:**
Provide comprehensive, health education and resource information details and support to patients/patrons (which may include Sutter employees) for the department’s individual and group services which are delivered both virtually and in-person. Maintain presence and assist patrons with health education and resource needs at designated Health Education Resource Centers (includes helping Walk-in Patrons, phone call and email follow up. Regularly review Provider generated Patient via EPIC. Investigate and provide individual patient information, and resource needs via My Health Online messaging, telephone conversations, and scheduled in-person visits. Engage patients/patrons in motivational coaching sessions aimed at supporting development of personal reasons for healthy lifestyle changes (which may include Provider prescribed or recommended treatments) and support behavior change skills and effective goal setting. Evaluate requests for materials from a variety of evidence-based sources and gather quality information for distribution. Assist with virtual webinar/class and group facilitation. Design new class/group offerings, as needed. Participate in collaborative projects with department workgroups, including clinical areas. Present Health Education Department Services and offerings to Providers, staff, and a variety of department groups and community organizations. Tally patient/patron engagements, review and use data as a tool for setting team goals. **Job Description** :
**EDUCATION:**
Bachelor's degree preferred
**TYPICAL EXPERIENCE:**
2-3 years of recent relevant Health Education experience.
**SKILLS AND KNOWLEDGE:**
+ Mastery of basic computer skills (Outlook, Word, Excel, PowerPoint, Microsoft TEAMS. Additionally, Electronic Medical Record via EPIC)
+ Excellent interpersonal, organizational, written and verbal skills (including presentation skills)
+ Self-directed, able to set priorities and work independently
+ Familiar with medical terminology
+ Bilingual in Spanish or Mandarin Preferred

Pay Range is $44.97 to $58.46 / hour
_The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
