Patient Engagement Coordinator AIM

Here, you’ll join gifted colleagues who make a lasting difference in people’s lives every day. **Overview** We are looking for a Patient Engagement Coordinator to join our team and report directly to the Director of Patient Engagement. This role will coordinate initial and follow up outreach to patients for initial visit scheduling purposes. Remote position, with flexible schedule of 9am-6pm or 10am-7pm. + Perform telephonic outreach to patients + Maintain communication with pending patients, their families, AIM sites, and referral sources and routinely follow the progress of prospective patients and those referrals not taken under care (NTUC). + Coordinate and maintain effective and efficient scheduling for the Nurse Practitioners to include geographical considerations that may impact time management and company cost. + Ensure and verify appropriate data entry for all key components related to a patient referral, to include but not limited to: patient legal name, payor, address, DOB, and telephone number. + Maintain patient status updates in all required systems, reports and EMRs **About You** + High school degree or equivalent; College graduate preferred + Minimum of three years health care delivery or related business experience required + Strong knowledge of medical terminology and a customer service focus. + Experience in a role related to direct to customer interaction and engagement preferred + Effective data entry and word processing, problem-solving, human relations, and oral/written communications skills. + Experience with Computer applications and Electronic Medical Records. + Strong comfort level in performing telephonic outreach to patients whether referred from a warm hand off or a partner provided list.
