Major Responsibilities:

* Identifying patient eligible for grant criteria via access through the Emergency Department

* Data collection and Entry

* Building community relationships with outside referral sources

* Providing reliable transportation for patients as needed

* Providing case management and referrals to meet patient needs

* Connecting patients to the UMass Harrington Healthcare Behavioral Health continuum of care

* Providing follow up contact with patients while part of the Bridge Clinic

Standard Staffing Level Responsibilities:

1. Complies with established departmental policies, procedures, and objectives.

2. Complies with all health and safety regulations and requirements.

3. Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.

4. Maintains regular, reliable, and predictable attendance.

5. Performs other similar and related duties as required or directed.

All responsibilities are essential job functions.

Position Qualifications:

1. Experience with substance use disorders and mental health disorders preferred or willingness to learn.

2. Excellent communication skills and interpersonal skills.

3. Attendance to various staff meetings in accordance with grant requirements and to ensure program staff involvement.

4. Valid driver’s license and proof of insurance

5. Strong organizational skills

6. Willingness to work with a multi-disciplinary team.

License/Certification/Education:

Required: Bachelor’s Degree

Preferred: Certified Alcohol and Drug Counselor (CADC) or Licensed Drug and Alcohol Counselor (LADC) preferred

Experience/Skills:

Required: Experience with substance use disorders and mental health disorders preferred or willingness to learn.

Preferred: Two years working within the substance use disorders field
