Job Summary
The Patient Navigator for the BWFH Inpatient Addiction Recovery Program is responsible for working closely with patients, families, and healthcare providers to ensure a seamless and patient-centered care experience by assisting with navigating the complexities of the healthcare system, coordinating resources, and providing support to improve patient outcomes.The BWFH inpatient Addiction Recovery Program is a Level Four Detoxification program accepting up to 8 patients at a time, located on a medical floor within the acute medical hospital. The program provides appropriate medical care for managing a patient’s withdrawal from alcohol and other substances that require safe medical management. In addition to the medical management, patients receive individual and group counseling with master’s level clinical social workers around their substance use along with planning for patient-specific treatment options. This is vital to prepare them for their next stage of recovery when they are discharged from the program.
Qualifications
Duties & Responsibilities:
Ability to meet in person with patients on an inpatient addiction recovery unit to assist with designated tasks assigned by primary social work team as noted below.Work directly with each patient to request signature for release of information forms (42 CFR) as needed by social work or team to communicate on behalf of patient.Assist with placing referrals requested by patient and primary social worker to substance use treatment programs (faxing, calling, and follow up with programs to determine availability) as part of a patient’s after care plan when preparing for discharge.Assist with faxing the discharge summary to a PCP when a patient consents, and/or faxing discharge paperwork to treatment programs requesting them as part of acceptance into a program when a patient is being discharged.Join with social work and interdisciplinary team during daily interdisciplinary rounds to document in Epic (electronic medical record) during or immediately after rounds those who participated and note of progression (yes/no) in each patient’s treatment plan,Assist with printing out of a treatment plan for each patient at beginning of their admission and have patient sign to place in patient’s chart.Assist with other tasks as assigned by social workers providing direct patient care.Requirements (Knowledge, Skills and Abilities)
Minimum of Bachelor's Degree in Social Work, or Related Field of Study required.1-2 years of related work experience (social work, case management or related field preferably in a clinical setting) preferredExperience with care coordination and referrals to community resources, strong knowledge of healthcare resources, community services, and patient advocacy.Demonstrated commitment to respecting and impacting the care of high-risk patients.Excellent verbal and written communication skills.Excellent organizational and time management skills (ability to prioritize and follow through with tasks essential to a patient’s care needs as assigned).Ability to be flexible and resourceful.Assessment and problem-solving skills.Confidence and ability to work independently and as part of a diverse multidisciplinary team.Proficiency with Microsoft Office: Excel, Teams, and PowerPointFamiliarity with electronic health records and case management software.Familiarity with substance use treatment resources and mental health services as well as systems of care and community resources.
