Job Summary
The Patient Navigator enables the timely coordination of multidisciplinary care for Stroke and neurorecovery patients. This individual serves as the liaison among specialty services and works closely with Neurology, Neurosurgery, Emergency Medicine (Access Center), and other radiology services as needed to create an efficient and patient-centered system for patient evaluation, treatment, and follow-up.- Responsible for working closely with patients, families, and healthcare providers to ensure a seamless and patient-centered care experience by navigating the healthcare system's complexities, coordinating resources, and providing support to improve patient outcomes.
- Advocate for patients and their families, ensuring their needs and preferences are considered in the care planning process.
- Serve as a liaison between patients, healthcare providers, and other relevant stakeholders.
- Assists Stroke and Neurorecovery patients with scheduling clinic appointments, radiological studies, procedures, and/or surgical appointments for patients discharged from the hospital, Emergency department, or those seeking elective referrals.
- Collaborate with the interdisciplinary healthcare team to coordinate patient care services, appointments, and follow-up plans by triage protocols to ensure are seen by the right specialist at the right place and time.
- Assist patients in understanding and adhering to their care plans.
- Identify and connect patients with appropriate healthcare and community resources, such as support groups, financial assistance programs, and transportation services.
- Educate patients and their families about their medical conditions, treatment options, and self-care strategies.
- Ensure that patients are informed and empowered to actively participate in their healthcare decisions.
- Maintain accurate and detailed documentation of patient interactions, care plans, and resource referrals.
- Alerts leadership to patient service issues or clinical needs that need attention.
Qualifications
A bachelor's degree in public health, clinical social work, healthcare management, or a related field of study is required.
