Through this community of healers, scientists, and scholars, Rutgers is equipped as never before to transform lives Posting Summary Rutgers, The State University of New Jersey, is seeking a Patient Navigator for the department of Medicine at the Robert Wood Johnson Medical School.
This position provides patients with assistance in access to healthcare, maintenance of comprehensive treatment and case management services.
Among the key duties of this position are the following: Acts as a liaison for patients in the non-invasive cardiology practice, ensuring timely and coordinated access to care, including diagnostic testing, treatment options, and follow-up services. Supports patient access to care by assisting with attendance at evaluations, follow-up visits, consultations, referrals, and necessary documents (e.g., insurance pre-authorization, school & travel letters). Coordinates and monitors patient appointments, communicates with clinical staff regarding patient flow, and addresses any issues that arise. ’ Handles patient referrals and pre-certification numbers for procedures. Advises patients on the preparation and requirements of necessary forms and paperwork. Collaborates with the cardiology team to identify patients needing assistance, reminding patients of appointments, arranging transportation, and helping non-English speaking patients access translation services.
FLSA Exempt Grade 22S Salary Details Minimum Salary 60543.00 Mid Range Salary 72444.00 Maximum Salary 85865.00 Offer Information The final salary offer may be determined by several factors, including, but not limited to, the candidate’s qualifications, experience, and expertise, and availability of department or grant funds to support the position. We also take into consideration market benchmarks, if and when appropriate, and internal equity to ensure fair compensation relative to the university’s broader compensation structure.
