Patient Navigator

Position Summary: As a member of Signature Addiction Medicine (SAM) the Patient Navigator will assume responsibilities for the care coordination for patients with Substance Use Disorder(s). The Patient Navigator works collaboratively with internal and external entities in order to remove barriers to care and ensure smooth transitions for patients/families. The Patient Navigator works with patients to address and meet their needs by linking the patient to appropriate medical and social resources. Location: 360 Brockton Ave, Abington, MA 02351 Department: Signature Addiction Medicine/OBAT This is a full-time 40 hour/ week position Responsibilities: + Assist patients in referring/connecting to services and resources including addiction treatment, recovery support, healthcare and social supports such as job training, GED programs, housing, financial resources, food access and transportation. + Assists patients in various areas to help advance their care and reduce social drivers of health by connecting them to appropriate specialists, agencies and support systems. + Maintains relationships with community entities and provides appropriate outreach when needed. + Assists patients with completion of required/necessary paperwork. + Visit clinical sites in hospital to provide administrative support in scheduling appointments with patients, provide information to patient, obtain and verify contact information/consents, assist with care plan, etc. + Coordinates/removes barriers to transportation to appointments and services as needed. + Assists in resolving insurance issues and obtaining authorizations as needed. + Works with external referral sites to ensure proper referrals to and from the Signature OBAT Program. + Helps patients/families follow up with their medical appointments through phone call reminders. + Works with patients to provide the medical team with necessary information to help in their care. + Collaborates with provider and works closely with patient toward adherence to their care plan. + Helps patients/families solve daily problems that interfere with care. + Provides timely updates or changes in patient condition to the provider and other care team members. + Assists in data collection to monitor success for performance measures. + Assists in data analysis for reporting and measurement purposes. + Documents patient encounters in the electronic medical record in a timely manner. + Helps patients/families interact with community-based resources and services. + Follows up with patients who missed appointments or have fallen out of care. + Works collaboratively with the Signature Healthcare OBAT Program and other community recovery programs and agencies. + Maintain patient confidentiality. + Conforms to hospital and Signature Medical Group standards of performance and conduct, including those pertaining to patient rights, so that the best patient care may be provided. + Performs other duties as assigned or necessary Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment following our relationship based care model: + Commits to recognize and respect cultural diversity for all customers (internal and external). + Communicates effectively with internal and external customers with respect of differences in cultures, values, beliefs and ages, utilizing interpreters when needed. BASIC KNOWLEDGE/SKILLS/APTITUDE/EXPERIENCE: + Knowledge of addiction and addiction treatment, health care systems, and community resources. + Ability to exercise good judgment and communicate effectively, build good relationships, show compassion, empathy and understanding of persons with addiction, behavioral health disorders and trauma. + Ability to exhibit resourcefulness, initiative, and problem solving. + Ability to multi-task and work under time pressures. + Ability to read and interpret documents accurately, such as patient charts. + Ability to communicate well both verbally and in writing with other members of the clinical team. + Excellent English communication skills. Additional language skills appropriate to the patient population served (especially Spanish and/or Creole) is desirable. Education/Experience/Licenses/Technical/Other: + Education : High School graduate or equivalent required; Associates degree preferred. + Experience (Type & Length): 1 – 2 years of successful working experience in a similar capacity. Prior addiction or behavioral health clinical/program experience helpful and preferred. + Certification/Licensure: Must hold a current Medical Assistant Certification + Software/Hardware: Proficiency with Microsoft Office, general knowledge of patient registration systems and electronic medical records, basic computer skills to enter, interpret and extract medical information from electronic systems in the healthcare environment.
