Compensation varies based on factors including, but not limited to, experience, skills, education, performance and salary range at the time of the offer._ **Responsibilities** Coordinate access to health-related services by assisting clients with utilizing available resources. + Identify potential clients, complete initial need assessment, and maintain follow-up to ensure appropriate guidance to health services. + Provide informal counseling, education, and social support to clients. + Assist clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, and assisting with completion of medical program applications. + Develop and maintain strong working relationship with external (referral sources and community agencies) and internal contacts (departmental team members). + Maintain accurate client records by documenting all contacts, services provided, and outcomes in order to track all clients and services for future reference. **Requirements** Licensure / Certification / Registration: + BLS Provider preferred. American Heart Association or American Red Cross accepted. + Drivers License preferred.
