Responsibilities
Summary
The patient navigator has a unique role in patient care. As a patient advocate, this individual will build working relationships, solve problems, direct patients to resources both within the organization and externally in the community as well as support patients as they learn to self-navigate the healthcare system. The primary focus of the Patient Navigator is to support providers in multiple clinics coordinate the plan of care with the healthcare team. The RN Patient Navigator will assume a leadership role within the clinics for clinic personnel to prepare and assist them with their responsibilities.
Qualifications
At least 3 years in hospital or health care setting with accountability for working with patients in nursing and/or case management/care coordinationA RN LicenseAbility to listen attentively with empathy and capture essential information for continuity of careAbility to work collaboratively with all care professionals including physicians, nursing, and external community agencies and post-acute care facilities Ability to complete required data entry in a timely manner
