The Patient Safety Officer (PSO) is responsible for identifying patient safety events including near misses, precursor and adverse events, and completing review, analysis, and assessment of these events. The PSO develops strategies to mitigate or prevent patient harm. The PSO must be able to think critically, identify problems, and work cooperatively to develop and implement solutions. The PSO plays a key role in Crozer Health’s patient safety initiatives and in promoting a culture that emphasizes safety in all aspects of its operations.
Responsibilities (includes but not limited to)
· Investigates patient safety events entered through event reporting system (RL)
· Monitor reports of events and identifies significant trends
· External reporting to Pennsylvania Patient Safety Reporting System (PA-PSRS) and PA DOH
· Prepares confirmation letters regarding serious patient safety events
· Conducts ACAs, RCAs, and safety debriefings
· Provides patient safety education (i.e., New Employee Orientation, HRO training, and event reporting
· Performs other duties as assigned or required
Requirements
· Bachelor Degree: Nursing (Required)
· Masters Degree: Nursing or healthcare related field (Preferred)
· Valid PA Nursing License (Required)
· Certified Professional in Patient Safety (CPPS) or equivalent (Preferred)
· 5 years of experience in a hospital setting (Required)
· 2 years of experience Patient Safety, Quality, Performance Improvement, or Risk Management (Preferred)
· Experience with data collection, basic statistics and statistical measurement tools, and data analysis
· Excellent interpersonal, communication, presentation, and facilitation skills
· Strong planning, organizational, and change management skills
· Strong computer skills including working knowledge of Microsoft Office Suite: Word, Excel, PowerPoint, Teams (Required)
· Ability to effectively manage difficult situations
