*POSITION SUMMARY*
Under the direction of the Clinical Patient Safety Officer, the Patient Safety Specialist is responsible for the successful implementation and maintenance of an integrated patient safety management system as the foundation for a safety culture. The Patient Safety Specialist collaborates with medical staff, leaders, associates and patients to develop and implement strategies that identify and reduce risks, enhance safety, and meet regulatory requirements. The Patient Safety Specialist fosters a culture of continuous patient safety improvement that will sustain event reporting and reduction of errors and other risk factors that contribute to unintended adverse patient outcomes.
*PRIMARY ACCOUNTABILITES*
1. Facilitates patient safety activities and coaches’ leaders and teams in developing safety improvement capabilities using a process redesign and measurement approach.
2. Provides timely and accurate referral of human-factor concerns to appropriate leadership
3. Partners with department leaders and risk management to identify potential or actual safety risks and assists with the cause analysis and implementation of improvement plans to mitigate that risk. Provides direction and coaching to teams on organization-wide sustainment of safety culture.
4. Serves as a resource and subject matter expert for applicable Joint Commission standards as they relate to patient safety requirements. Facilitates the development of strategies to remain compliant with all safety regulatory requirements to include National Patient Safety Goals.
5. Rounds routinely in all clinical care departments to assess and address risk reduction and patient safety activities/concerns. Follow up with department managers and leadership regarding patient safety concerns as noted during rounding to ensure risk mitigation.
6. Participates in/leads entity and system patient safety huddles, committees, workgroups and Serious Safety Event reviews as needed. Serves as a patient safety consultant to various other committees to ensure integration of patient safety work across the continuum of care.
7. Analyzes trends of key performance metrics, identifies opportunities for improvement, develops and maintains performance improvement plans (PIP), measures success of the PIPs, and creates the follow-up metrics for each activity to maximize success.
8. Addresses results of safety culture survey and develops recommendations for appropriate action plans as needed
9. Creates and coordinates educational programs for new employees and providers regarding patient safety strategies. Also provides ongoing patient safety educational offerings for all associates.
10. Assists in coordinating and facilitating the FMEA (Failure Mode Effects Analysis) process and other patient safety tools such as patient risk assessments, as appropriate.
11. Assists in the preparation of patient safety reports to be presented at various committees and board meetings.
12. Manages sensitive patient and associate safety information in a professional manner and strives to always achieve excellence in coworker communication and collaboration.
*MINIMUM QUALIFICATIONS*
* Education: Bachelor’s degree in Healthcare or relevant field.
* Work Experience: Five (5) years in Healthcare.
* Licensure: If required for field work and current as applicable to Healthcare field (i.e., Nursing, Pharmacy, Lab, etc.).
* Certification: Certification in Patient Safety within one (1) year of start date.
* Skills/Knowledge/Abilities:

o Strong verbal communication skills.
o Strong motivational interviewing and assessment skills.

o Must be able to prioritize, plan, and handle multiple tasks/demands simultaneously.
o Ability to function independently and efficiently in various clinical environments and meet defined goals.
o Demonstrate computer application skills using tools such as EMR, MS Excel, Word, Outlook, and PowerPoint.
o Proven ability to accurately analyze, report and act on data.
o Must have the ability to maintain confidential organizational knowledge.
o Proficiency in using and teaching the use of safety event reporting tool.
* PHYSICAL REQUIREMENTS*
* Physically agile; may require walking, standing, running, bending, stooping, kneeling, climbing, or crouching frequently.
* May occasionally include lifting or moving objects up to 50 pounds, with or without assistance.
* May require moving in different positions to accomplish tasks in various environments including tight and confined spaces, including reaching out and above shoulders, or overhead.
* May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to 90% of the assigned shift.
* May be exposed to inside and outside environments with varied temperatures, air quality, lighting, and/or low to loud noise.
* May require working with biohazards such as blood/body fluids and airborne particles following OSHA and CDC guidelines.
* Communicating with others to exchange information.
* Visual acuity and hand-eye coordination to perform tasks.
* Workspace may vary from open to confined, onsite or remote.
* May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
