Care APC clinic is located at Jefferson St Hartford Hospital APC averages over patient visits annually with an average of patients cared for per day We see patients for preventative annual care as well as patients with wide range of health problems such as diabetes high cholesterol asthmaCOPD and hypertension Staffing includes attending physicians APRNs Residents Psychologist Social Worker Nutritionist Pharm D RNs MAs and MAAs APC facilitates an ongoing relationship between patients and clinicians and fosters participation by patients in decision making about their health and their own care of our patients are bilingual with Spanish being their primary language The majority of our patients have health coverage under Medicaid APC provides opportunities for disease prevention and health promotion as well as early detection of problems APC guides patients through the health system including appropriate referrals for services to other health professionalsJob SummaryThe Patient Service Coordinator is responsible for providing outstanding and professional customer service to ensure operational efficiency and promote a positive patient experienceJob Responsibilitiesmiddot Greets and registers arriving patients completes registration forms and verifies insurance including scanning and faxing forms as necessary middot Answers incoming calls evaluates priority and directs calls according to the urgency and subject mattermiddot Mails information to new patients prior to visit in accordance with department policies and enters updates patient demographicsmiddot Schedules new patient appointments and informs patients of essential preparation requirements prior to visit per department policies referrals x rays medical reports etcmiddot Ensures that office appointment schedule is coordinated with clinical staff activitiesmiddot Reviews assigned appointment confirmations to ensure accurate schedule and appropriate communications with patients completes appointment confirmation activities according to department policiesmiddot Schedules follow up appointments Verifies insurance coverage and obtains authorizations middot Orders department suppliesmiddot Maintains schedule for conference roommiddot Maintains a clean waiting room and office areamiddot Defines and resolves patientphysician issuesproblems bringing concerns to team lead or manager as necessarymiddot Completes facility billing according to facility level criteria Refers payment and insurance problems to Billing Department middot Provides back up support to other clerical staff and
