Provide patient-centered, coordinated access to the complex network of facilities, institutes, and clinicsFoster and nurture relationships with existing patients and those seeking treatmentProvide support related to Care Access including but not limited to:Providing support to patients in a high volume omni channel environment via phone, email and chatFind a physician navigation within a health networkScheduling appointments and classesProviding tier 1 technical support with patient health applicationsServe as an entry point and informational resource for patients into the health systemProvide high-touch, personalized assistance to patients across Providence St. Joseph Health regions and provider networksPlease note the following important details regarding this position:
