Summary :The Patient Services Specialist works closely with the homecare and hospice interdisciplinary clinical teams to support the patient flow along the care continuum. Coordinates the initial and ongoing scheduling needs for skilled care services while collaborating with other ancillary departments to ensure a timely, efficient, and seamless delivery of care.

Job Description:

KEY ACCOUNTABILITIES:

Reviews referral information to determine patient’s home medical needs for all new and existing skilled care referrals. Provides initial contact to patient/family and clinical team members.

Initiates, coordinates, and documents pertinent and current medical information to ensure efficient care coordination for requested homecare and hospice services.

Collects comprehensive information from referral source and/or patient/family that supports timely and appropriate care needs.

Interfaces with other departments and disciplines to ensure all requests for services are coordinated in a timely and professional manner.

Performs coordination of patient needs that support clinical, payer, and regulatory compliance.

Coordinates complex patient needs to right level of clinical care support that is timely and efficient.

Documents pertinent and current patient information and enters in to the electronic medical record as appropriate.

Collaborates during daily huddles with clinical disciplines to ensure proper load leveling of available clinical resources to meet the needs of the patient’s plan of care.

QUALIFICATIONS:

High School diploma or GED preferred

Must be 18 years of age

One year or experience with customer service

PHYSICAL DEMANDS:

Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance

Job classification is not exposed to blood borne pathogens (blood or bodily fluids) while performing job duties

WORK ENVIRONMENT:

Normally works in climate controlled office environment

Frequent sitting with movement throughout office space

Use of computers throughout the work day

Frequent use of keyboard with repetitive motion of hands, wrists, and fingers

Schedule- Saturday/Sunday position 8 hours both days. 8a-4:30p

Possibility to be remote/hybrid after 4-6 weeks of training on site.
