Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. Job Summary: Hours: Per Diem; As needed, Monday-Friday, 8:00am - 4:30pm Union:   No The Admissions RN has administrative and clinical responsibility for screening referrals and assessing appropriateness of admissions to the Respite Program.  Responsibilities include ensuring that all referrals are responded to in a timely and professional manner and the admissions are planned and coordinated with the appropriate administrative and clinical staff.  Reports directly to the Admissions RN supervisor. Responsibilities: + Is responsible for admissions coordinated during regular business hours Monday through Friday + Coordinates Admissions to the Respite Programs (Barbara McInnis House) + Screens admission referrals Monday through Friday.  Gives report to the RN Manager at the end of the day + Works collaboratively with BHCHP staff to facilitate the screening and admission process + Informs the clinical staff of planned admissions + Enters patient information into the computer + Greets patients, initiates a brief history, physical assessment, and documents the encounter in the patient’s medical record + Provides patient orientation to the Respite programs + Transports the patient to the appropriate floor and gives report to the Team Nurse and Provider + Problem solves with the Admissions RN manager regarding alternatives for those patients who do not meet criteria for admission to the Respite programs + Uses the electronic medical record to inquire about pertinent information related to referrals + Attends and participates in meetings as requested by the Admissions RN manager Qualifications: + Commonwealth of Massachusetts RN license + Minimum of 3 - 5 years nursing experience preferred + Previous hospital experience preferred + Demonstrated interest in working with an underserved population + CPR Certification required + Ability to communicate effectively orally and in writing + Ability to solve complex problems and deal with a variety of issues + Ability to effectively present information and respond to questions from staff and patients + Proficient in Spanish or Haitian Creole language; preferred + Computer skills, including e-mail, word processing, work scheduling systems; EPIC and MatrixCare experience a plus + Strong organizational skills Compensation and Benefits: + The compensation starts at $37.82 per hour and increases based on years of experience.
