Description:

This position exists to provide comprehensive, value based, medication-related care to achieve definite outcomes that improve a patient's quality of life. The incumbent acts in collaboration with healthcare professionals in the shared responsibility for patient drug therapy outcomes. Key job duties may consist of ensuring safety and effectiveness of the medication use process, processing and dispensing medications per order or prescription, medication therapy monitoring, regulatory compliance, maintaining oversight over pharmacist extenders, maintaining the security of medications, and protecting the confidentiality of patient information. Additionally, is competent at providing drug information support for medical staff, ensuring continuity of care, and identifying potential challenges in common situations.

Responsibilities:

Therapy/protocol review Medication, side effect, and toxicity management Collaborate with multidisciplinary teams Help develop plans for relapsed/refractory patients Dose adjustments for chemotherapy Education patients and families on new therapies and supportive care measures Participate in transitions of care and assist in high-acuity situations

Program Distinctions:

Early access to clinical trials through the Children's Oncology Group Phase I and Pilot Consortium

• Requires current Indiana License or eligibility for Indiana licensure as a pharmacist.
• PGY2 in Pediatrics or PGY2 in Oncology is required OR 5+ years experience in clinical pediatrics or oncology
