+ Works as consultant, educator, or community liaison to facilitate collaborative coordination of patient care
+ Responds to patient and family needs and acts as an advocate
+ Monday-Friday schedule with flexibility

**Qualifications:**
+ Education: Bachelors' Degree
+ Experience: 2 years' experience
+ Licenses/Certifications: Valid Driver's License and Auto Insurance
Experience preferred in social work, developmental disabilities, child protection, care coordination in a medical system, interacting with a care team to include doctors, nurses or school systems and IEPS
