Job Summary
SummaryClinical Social Work/Care Coordination
40 hour/week position divided between direct billable mental health and care coordination services.
Work directly with pediatric patients (and their families) in the outpatient rehabilitation setting who are injured or disabled, ill, or newly diagnosed with a pediatric condition and who may experience life altering changes in income, health insurance coverage, physical access to their environment, specialized transportation, and caregiver support to maximize independent function in the community. Collaborate across the continuum of care and with the interdisciplinary rehabilitation teams, community providers, and community agencies to promote the highest level of independent function and an active, productive, and fulfilling life. Direct and assist patients and their families in the application process for federal, state, and private resources and provide guidance to specialized community resources.
Additionally, the ideal candidate will be experienced in assessing and providing therapy to patients with diverse clinical conditions including anxiety, depression, OCD, ADHD, Autism, Attachment, trauma (PTSD), and often within the context of complex family systems and/or medical compromise.
Review daily referrals and screenings of clients.

Experience
1-5 years of Pediatric Counseling and/or case management experience preferred
Knowledge, Skills & Abilities
Highly developed communications and interpersonal skills, working with diverse population.
Work independently, be self-directed and flexible, and contribute as a member of a team.
High level of service delivery. Demonstrate initiative with ability to prioritize workload, meet deadlines and adapt to changing situations.
General working knowledge of available community resources.
Ability to complete documentation in adherence to department and/or payer standards.
Ability to establish strong rapport and relationships with patients and staff.
Proficient in Microsoft Office and industry related software programs.
Understanding of diagnostic criteria for dual conditions and the ability to conceptualize modalities and placement criteria within the continuum of care.
