As a key member of our behavioral health staff, this position will make significant difference by helping behavioral health clients engage in treatment and wellness. By sharing one’s own successes and challenges on the road to recovery this peer role supports clients and their recovery. The peer patient engagement specialist is an individual with lived experience who reaches out daily to clients who need additional support to stay engaged in treatment. Under the general supervision of the director, will serve as point of contact to patients, their families/representatives, outside agencies, and referrers. The peer engagement specialist will work to engage patients in their care, make daily appointment reminder contacts and follow up calls to patients who no show or cancel appointments. This position will assist patients in finding resources to reduce barriers to care and facilitate/schedule appointments. The engagement specialist candidate must demonstrate knowledge of Sheppard Pratt programs and services in interactions with internal and external groups. Also, will be required to enter required information into electronic medical record.
Principal Responsibilities and Duties: Actively seek to engage patients in treatment by providing reminder calls and follow up calls to reschedule patients. Document all contacts. Develop and maintain good working relationships with patients. Solicit and share patient feedback with Director and Request appropriate after care, discharge, and foster care information. Document required forms in the electronic medical record. Use on-line scheduling system to schedule patient follow-up appointments. Generate reports as requested. Assist patients in finding resources to reduce barriers to care. Engage patients in their care. Demonstrate understanding of the needs of client population (age and diagnosis) in interaction with clients. Support clients in promoting recovery and self-direction by appropriately sharing previous current personal experiences in mental health treatment. Assist team in follow-up process with hospitals and other service providers by obtaining releases of information and discharge summaries. Other duties as assigned. Knowledge, Skills, and Abilities Required: Work requires knowledge of community resources in social, medical, and mental health areas; knowledge of managed care organizations; basic knowledge of health insurance terminology, and basic oral and written communication skills normally acquired through the completion of a High School Diploma/equivalent and (2) years of direct job related experiences. Work requires interpersonal skills to communicate effectively with clients, all levels of internal staff, and various outside agencies and contacts. Must be a person in recovery from a behavioral health condition. Ability to keep appropriate boundaries with clients so that relationships are work related only Work requires analytical skills sufficient to edit documents, generate reports and logs, and summarize quality monitors. Excellent written and verbal skills for phone calls, letters, and documenting in the electronic medical record. Previous experience working in medical office preferred. Previous experience working with customers, clients or patients required. Must be proficient in the use of personal computer with Windows software. Analytical ability sufficient to gather and interpret data, apply knowledge of Health System programs to patient needs, investigate and resolve patient concerns and questions, and handle designated administrative detail. Knowledge of Recovery and Wellness. Peer Specialist training preferred but not required based on experience.What you'll get from us.
