Peer Recovery Specialist

Position Summary

Accountable for assisting consumers in the Emergency services/Screening program including: ensuring the coordination of patient care through follow-up with consumers, Psycho-education, assisting patients with linkage to treatment, other services & entitlements. Works collaboratively with the Emergency Services staff, 2 Central, psychiatry, Primary care Physicians, HBH and community outpatient treatment programs. Supports the mission, values and philosophy of HMC.

Primary Position Responsibilities

1. Respond in a timely manner when called to the emergency department or community.

2. Gather consumer information for follow-up and as required by grant(NJ Div. of Mental Health and Addiction Services)

3. Educate survivors on OORP program and navigating the treatment system

4. Be a positive role model for consumer and family.

5. Assist consumer in constructing a Wellness Recovery Action Plan.

6. Provide telephone and face to face follow up with consumers.

Work Contact Group (Internal/External)

HBH and PAH staff, Emergency Department staff, other Hunterdon Healthcare departments, State of New Jersey DMHAS, Hunterdon County Probation, Court system, other healthcare systems, agencies, patients and their Families.

Qualifications

Minimum Education:

Required:

High School diploma.

Preferred:

Bachelor’s Degree in Social Work, Psychology, or Equivalent.

Required:

● Excellent interpersonal communication skills.

● Ability to identify and resolve problems.

● Computer literate, skilled in customer service.
