The Peer Support Liaison acts as a liaison with peer support specialists in the community and related stakeholders. The Peer Support Liaison is a self-identified person with lived experience with a mental health condition and/or substance use disorder who is in successful and ongoing recovery from mental health and/or substance use disorders. The Peer Support Liaison uses their lived experience to support another person’s exploration of recovery-based services that can help them overcome the impact of mental illnesses or substance use disorders. Ensures recovery and resiliency principles as well as the perspective of our members and families are actively considered in every phase of the care management process.
Supports clients in navigating healthcare systems, advocating for their needs, and connecting them with relevant services.
Works closely with therapists, case managers, and other healthcare providers to coordinate client care and ensure a holistic approach.
Provides individuals with support and guidance on recognizing the early signs of crisis, developing crisis plans, and implementing identified coping strategies
Acts as an advocate for individuals to facilitate positive changes in their recovery
Empower
Maintain up-to-date knowledge of peer support specialist services in Virginia and related federal and state laws.
Establish and maintain strong partnerships with community and advocacy organizations, including NAMI, Mental Health American Chapters, community-based, and peer run organizations.
Support Care Managers in understanding when and how to engage peer support specialists in member care.
Identify opportunities to better engage peer support specialists in plan activities, including care management.
Collaborate with Care Managers, providers, and community agencies and organizations to identify those who may benefit from peer support.
Assist in facilitating access to and transition between services for members.
Develop and implement internal and external recovery based educational programs and trainings.
Serve in State Peer/Recovery Workgroup and/or serve as a member of internal committees focused on recovery and resiliency.

Required Qualifications
Possess active/current and unrestricted Peer Support Specialist State Certification
1+ year of working knowledge of community resources
Demonstrated understanding of the principles of recovery and resiliency
2 - 4 years of Microsoft Word, Excel, Outlook experience
Ability to facilitate trainings
Administration support experience in a healthcare industry with familiarity with care and well-being resources
Must reside in the Commonwealth of Virginia.
Occasional travel to office for staff meetings, training and member visits
Preferred Qualifications:
Bilingual preferred (Spanish, Arabic, Vietnamese or other)
Associate's Degree or Bachelor’s Degree in health and human services field
5+ years of demonstrated leadership experience

Workstyle: This is a remote position that will require you to travel.Travel: Up to 50% of the time to Humana Healthy Horizons office in Glen Allen, VA for collaboration and face to face meetings as well as field interactions with staff, providers, members, and their families.Workdays and Hours: Monday – Friday; 8:00am – 5:00pm Eastern Standard Time (EST).Language Assessment Statement: Any Humana associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency.
