**Employment Type:** Part time **Shift:** Day Shift **Description:** RN Intake - Per Diem - Eddy SeniorCare (Latham) Eddy SeniorCare has a rewarding opportunity for an RN to work out of our 385 Watervliet Shaker Road, Latham location! PACE is a unique program on the forefront of trends in healthcare, combining an insurance role (we are the Medicare/Medicaid insurer for our participants) with a comprehensive healthcare provider role (primary care, home health, adult daycare, transportation, all coordinated by an interdisciplinary team). This position is a community based RN role responsible for the following: + Initial intake, 6 months and annual UAS assessments of all enrolled PACE participants + RN assessment to include UAS, determination of support needs and collection of information to assist in determination if the participant continues to meet eligibility criteria + Identifies risks for avoidable ER and hospital utilization and communicates promptly to the RN Case Manager and PACE primary care provider. + Updates RN Case Manager regarding changes in participant needs in conformance with Agency policies and procedures, State and Federal regulations, and professional, legal and accrediting body standards of practice. + Assists the patient in achieving his/her optimal level of health and function. + Assesses, plans, implements and evaluates patient care using the nursing process. Pay Range: $36.50 - $49.95 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
