Job Highlights:

This is a part-time position working 20 hours a week Mon-Fri between the hours of 8am-5pm. Both 4 and 8-hour shifts available.

MUST have a masters degree in social work and have a current Missouri license of LMSW or LCSW.

Job Summary:

Provides case management and care coordination to pregnant women with substance use disorder throughout thier pregnancy and up to two years after the birth of the child.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

Connects the mother with reliable referrals in the community to support meeting the basic biopsychosocial needs of mother and infant.Connects mothers to health systems and community organizations for well-baby checkups, meeting development milestones, as well assessing for a safe and healthy home environment.Builds a trusting relationship with the mother and provides coordination between inter-disciplinary team, health systems and community organizations in meeting the medical needs of mothers and their children are also receiving their well-baby checkups, meeting development milestones, as well as have a healthy home environment for the infant to thrive.Works closely with Children's Division to coordinate disposition of baby upon discharge from hospital.Develops individual plan of care in coordination with the inter-disciplinary team that focuses on client needs through evidence-based interventions.Maintains documentation of activity, programs and participation.Collects and evaluates data to reach and maintain goals of program.Works in a constant state of alertness and safe manner.Performs other duties as assigned.

PHYSICAL REQUIREMENTS

Frequent reaching, sitting, standing and walking.Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more).Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.Frequent keyboard use/data entry.Occasional bending, stooping, climbing, twisting and gripping.Occasional lifting/carrying and pushing/pulling objects weighing 0-25 lbs.Occasional use of vision to identify and distinguish colors.Occasional driving.Rare repetitive foot/leg and hand/arm movements and squatting.
