Right now we’re looking for a Temporary Full Time Pharmacist, Drug Usage Evaluation to work in Pharmacy Services.
The Drug Use Evaluation (DUE) Pharmacist is responsible for the promotion of rational selection and optimal use of medications by planning, implementing and monitoring the Drug Use Evaluation Program within the hospital

Hourly Rate: $50.62 - $67.73

Site: Wilson (may be required to travel to other sites)
Hours: Monday - Friday Days (subject to change)
Responsibilities
Develops and implements a formalized Drug Use Evaluation (DUE) Program, with consideration of both Clinical Program and Pharmacy Services business plans and objectives. Monitors medication utilization and expenses by therapeutic classifications/Expense Object Codes (EOC) and patient care units on a regular basis to identify areas of sub-optimal use and works with the Pharmacy and Therapeutics Committee, Program and Physician Directors, Medical Chiefs/Specialists, and Clinical Pharmacists for resolution.Reviews prescribing patterns with reference to appropriateness, clinical outcomes, and adverse reactions in concert with Programs and Pharmacy Services.Co-ordinates medication utilization review efforts by the Programs and Clinical Pharmacists.Participates in and/or co-ordinates audit criteria, data collection, analysis and interpretation of retrospective or prospective DUE studies or drug expenditures with pharmacy, nursing and medical staff; for presentation to the Pharmacy and Therapeutics Committee and/or other appropriate committees/groups as necessary.Participates in the Antimicrobial Committee, Safe Medication Practices Committee and other committee meetings, as neededParticipates in and assists in coordinating the agenda and minutes for the Pharmacy and Therapeutics Committee.Prepares the electronic newsletter on “Formulary News and Update” on a regular basis for distribution to all medical, nursing and pharmacy staff.Establishes and maintains a system to document and analyse savings and effectiveness of medication utilization review efforts.Makes recommendations to improve any aspect of medication use, including drug administration systems and prescribing patterns.Develops the hospital formulary by preparing and maintaining the Formulary & Drug Use Manual and Parenteral Drug Manual, in collaboration with the Director, Pharmacy Services. Promotes adherence to the formulary and related drug use policies, guidelines and protocols. Assists in the development and implementation of drug related guidelines and procedures such as automatic substitution/therapeutic interchange policies where cost savings may be realized.Provides drug information as requested and educates medical, nursing and pharmacy staff about effective drug use, as part of implementation of drug use guidelines and protocols.Submits Drug Utilization Evaluation results and outcomes for publication when the opportunity arises.Networks with and exchanges ideas with other DUE Pharmacists by attending the DUE Professional Specialty Group (PSG) meetings.Acts as resource for clinical pharmacists.Establishes and maintains Adverse Drug Reaction program and reports to Pharmacy & Therapeutics Committee regularly and externally where appropriate.Alerts prescribing staff and other healthcare staff of any drug shortages and the potential therapy alternatives/management or temporary guidelines/procedures needed.Works with pharmacy inventory staff members in order to address inventory selection and medication selection matters. Participates in drug distribution activities as the need arises.Maintains an awareness of departmental and hospital continuous quality improvement programs, and participates in related activities on request.Provides clinical pharmacy services to clinical programs, as required.
Qualifications
Current licensure with the Ontario College of Pharmacists - Part AAdvanced degree in Clinical Pharmacy or hospital residency programs an asset.Minimum of 5 years hospital experience in clinical pharmacy practice required.Ability to design and implement data collection systems.Computer skills including pharmacy systems software, business applications such as spreadsheet, word processing and database programs required.Demonstrated ability to work independently and proactively, showing initiative.Demonstrated written and verbal communication as well as presentation skills.Ability to teach and support staff.Ability to prioritize workload.Ability to work effectively in a team setting in a collaborative manner.Demonstrated effective judgement and problem solving skills.Customer service and quality focused.Hospital Pharmacy experience with strong patient oriented clinical background and substantial knowledge in pharmacotherapeutics as well as pharmacoeconomics required.Drug utilization evaluation and organization skills are required.Experience in drug use evaluation and drug information roles preferred.Highly developed critical thinking, practical problem solving, and organizational skills.Flexibility required to effectively work in a busy diverse environment while balancing multiple competing/changing priorities and deadlines.Demonstrated initiative in continuous quality improvement and LEAN processes Holds self and others accountable.Strong conflict resolution and negotiation skills, an assetFamiliar with federal and provincial drug schedules.Excellent attendance and discipline free record required.
